Hello: I have several questions about sacrament notations on the back of Baptismal Certificates. I have recently noticed that neither my baptismal certificate nor those of my young adult children have these notations. Because my children are now at the point of moving out of the family home, it seems to me it would be important to have the forms, but I am curious about the notations.
